To provide healthcare services and manage our practice, we collect personal and health information including your name, contact details (including emergency contact), date of birth, Medicare/DVA and private health insurance details, medical and family history, relevant genetic information, medications, investigation results, referral information, payment and billing details, and clinical notes.
This information is collected directly from you through registration forms, questionnaires, consultations, telephone calls, emails, SMS, and online enquiries. Where relevant to your care, we may also obtain information from your referring doctor, other healthcare providers, hospitals, pathology and imaging services, Medicare, private health insurers, or government agencies.
How Your Information Is Used
Your information is used to provide healthcare services, coordinate referrals and investigations, communicate results and treatment recommendations, manage appointments and billing, and meet legal, professional, and accreditation requirements. Access to your information is limited to authorised staff involved in your care or clinic operations.
Sharing Information with Others
Where necessary for your treatment, we may share relevant information with your GP, referring practitioner, specialists, hospitals, allied health providers, pathology and radiology services, Medicare, private health insurers, and other organisations involved in your care. Information may also be disclosed where required by law, to prevent a serious threat to health or safety, or where you have provided consent. We do not sell patient information to third parties.
Use of Artificial Intelligence (AI) Clinical Documentation Tools
The Toowoomba Breast and Menopause Clinic utilises AI transcription software during consultations to assist with accurate documentation and correspondence. No audio recordings are retained, and all transcripts, clinical notes, and letters are generated in accordance with our privacy obligations and Australian patient privacy standards. Electronic Communication
We may contact you by telephone, SMS, email, or secure messaging regarding appointments, recalls, results, investigations, and other healthcare-related matters. While we use secure systems where possible, electronic communication may carry inherent privacy and security risks.

Your information is stored securely in electronic and/or physical records. Access is restricted to authorised staff, and reasonable safeguards are in place to protect your information from unauthorised access, loss, misuse, or disclosure.
